What is Proxy Access?
Proxy access lets someone else (a parent, family member, or carer) manage a patient’s GP online services on their behalf. This can include:
– Booking and cancelling appointments
– Ordering repeat prescriptions
– Viewing parts of the patient’s medical record
The proxy (the person managing the account) has their own login details. They do not use the patient’s personal login.
When is Proxy Access Appropriate?
Proxy access may be useful when:
– Parents or guardians manage the healthcare of children (subject to age and confidentiality rules)
– Carers help look after an adult who cannot manage their own care
– Family members support older relatives with prescriptions and appointments
How to Apply
To apply for proxy access, you need to complete a request form and provide identification.
– Download the Proxy Access Application Form
– Or collect a paper form from reception
Once completed, return the form with ID to reception. We will review your request and set up access if appropriate.
Important Information
– The GP will review each proxy access request carefully to ensure it is in the patient’s best interests.
– Patients can withdraw proxy access at any time.
– Proxy access may be limited (e.g. prescriptions only) depending on the patient’s needs and wishes.
