Child Registration 0-12YRS

To register a child under 12 please bring their RED BOOK which will have the NHS number inside. You as the parent must also be registered at our practice.

If you wish to register your child at The Earlsfield Practice, you can either come to the surgery to collect a registration form and bring it back to us or you can email us on [email protected] and ask that we send you a registration form that you can fill out electronically and send back to us, which will be quicker.

Alternatively, if you prefer to collect the registration form then we ask that you fill the form in BLOCK CAPITALS to prevent any delay.

Although we strive to complete every registration received on the same day we kindly ask that you do allow up to 2 working days for the registration process to be complete.

We accept New Patient Registrations between Monday to Friday 08:00  to 18:00

We advise for new born babies to be registered as soon as possible, so that you can book in advance for your babies 8 weeks check and immunisations.

It has been a pleasure serving you and the local community during the past 37 years as a GP. With a certain degree of sadness, I have decided to retire at the end of August this year, 2021. Dr Bamford and Dr Salim will remain partners at the Earlsfield Practice and a new GP partner will be appointed in due course. You do not need to do anything regarding your registration. The Earlsfield Practice staff will continue to provide care for all those who are registered at the practice.
